History: Constituted as 445 Medical Group on 24 June 1952. Activated in the Reserve on 8 Jul 1952. Redesignated as 445 Tactical Hospital on 18 May 1954. Ordered to Active Service on 28 Oct 1962. Relieved from Active Duty on 28 Nov 1962. Discontinued, and inactivated, on 11 Feb 1963. Redesignated as 445 USAF Clinic (Associate) on 18 Oct 1982. Activated in the Reserve on 1 Dec 1982. Redesignated as 445 USAF Clinic on 1 Apr 1993. Inactivated on 1 May 1994. Redesignated as 445 Medical Squadron on 9 Sep 1994. Activated in the Reserve on 1 Oct 1994. Redesignated as 445 Aerospace Medicine Squadron on 1 Jul 2000.
