Redesignated as 445 USAF Clinic on 1 Apr 1993. Inactivated on 1 May 1994. Redesignated as 445 Medical Squadron on 9 Sep 1994. Activated in the Reserve on 1 Oct 1994. Redesignated as 445 Aerospace Medicine Squadron on 1 Jul 2000.
